Tonsillitis without a sore throat: what to know

Tonsils can look enlarged or have patches even when throat pain is minimal. Some people notice bad breath, a change in voice, a sensation of something stuck, swollen neck glands, ear discomfort, or difficulty swallowing instead. Children may show reduced appetite or irritability rather than describing pain clearly.

The appearance may accompany a common cold, influenza, COVID-19, glandular fever, allergic rhinitis, or another throat irritation such as acid reflux. Strep throat and tonsillitis remain possible, but the visible appearance does not identify one cause and does not mean that a serious condition is present.

Check whether you can drink, eat, breathe, and open your mouth normally. Maintain fluids and gentle oral hygiene, and avoid smoke or other throat irritants. Record new symptoms and their timing. A clinician can examine the tonsils and surrounding tissues if the change persists, recurs, or is accompanied by swallowing difficulty or gland swelling.

Seek urgent care for breathing difficulty, drooling, inability to swallow fluids, severe one-sided swelling, a muffled voice, or difficulty opening the mouth. Arrange routine medical advice for persistent enlargement, repeated episodes, unexplained weight loss, or a neck lump that does not settle. Do not wait for pain to appear if the swelling is worsening.

Ask whether the tonsil appearance is typical for infection, whether another condition could explain it, whether testing is needed, and which changes would require urgent review.
