Fungal nails may become discoloured, crumbly or thick and can occur with athlete’s foot. Repeated pressure from footwear or an old injury can distort one nail. Nail psoriasis may cause pitting, separation or colour change as well as thickness. These patterns can overlap, so thickness alone does not prove fungus.
A thick nail can press against shoes and surrounding skin, causing tenderness or making trimming difficult. The altered portion grows out slowly; filing the surface may improve comfort but does not treat an underlying infection or inflammation.
What to do
Keep the nail clean and dry, trim it carefully after bathing when it is softer, and use roomy footwear. Do not force tools under the nail or share clippers. Treat athlete’s foot promptly if present and disinfect personal nail tools after use.
A clinician may recommend a nail sample before antifungal medicine. Oral antifungal treatment is considered only after reviewing health history and medicines. Laser treatment may be discussed in selected situations, but it is not a substitute for confirming the diagnosis.
