Understanding nappy rash

Nappy rash develops when moisture, urine, stool, and rubbing irritate the skin under the nappy. Redness may affect the buttocks, genitals, or skin folds. A rash in the folds with small nearby spots can have a yeast component, whereas irritation often follows prolonged dampness or diarrhoea. New wipes, soaps, detergents, or medicines can also trigger a reaction. The appearance alone cannot identify every cause, so worsening or unusual rash deserves an examination.

Comfort and skin care

Change nappies promptly after wetting or soiling. Rinse with lukewarm water or use gentle, fragrance-free wipes, then pat rather than rub. Allow the area to air briefly and apply a thin, even layer of a bland protective ointment at each change. Give supervised nappy-free time when practical, and choose a comfortably loose nappy. Avoid talcum powder, fragranced products, scrubbing, and home remedies that sting. Emollient therapy may support a damaged skin barrier; a clinician or pharmacist can advise whether an antifungal treatment is appropriate when yeast is suspected.

When to seek care

Contact a healthcare professional if the rash is severe, bleeding, blistered, crusted, oozing, spreading beyond the nappy area, or not improving with careful home care. Seek urgent advice if the baby has fever, seems unusually sleepy, feeds poorly, or appears very unwell. Do not use steroid or antifungal creams without guidance for a young child, because the correct product and duration depend on the diagnosis and site.
