What should you know about threadworm itching?

Threadworm is an intestinal parasite; its characteristic symptom is itching around the anus that is often worse at night. Some people have few other symptoms. Itching elsewhere on the body, a visible rash, dry scaling, raised wheals, or irritation after a new product may point to another cause rather than threadworm.

Possible skin explanations include asteatotic eczema, atopic dermatitis, contact dermatitis, skin yeast infection, lichen planus, psoriasis, scabies, or hives. These conditions do not all look or behave alike, so scratching alone cannot identify the cause. A clinician may ask where the itch is, when it started, whether anyone else at home is affected, and whether there are bowel or sleep changes.

Arrange a medical or pharmacy assessment if the itch is persistent, disturbs sleep, or is focused around the anus. Treatment for threadworm is different from treatment for eczema, allergy, scabies, or yeast, so avoid using leftover creams or repeated antiparasitic medicine without advice.

Keep nails short, wash hands after using the toilet and before eating, and avoid sharing towels while the cause is being clarified. A cool compress and fragrance-free moisturiser may soothe irritated skin, but stop any product that stings or worsens the rash. Ask whether household members need assessment if threadworm is suspected.

Seek prompt care for rapidly spreading rash, facial or throat swelling, breathing difficulty, pus, fever, severe pain, or skin that is blistering or breaking down. Arrange review for itching that continues despite a sensible care plan, causes repeated skin damage, or is accompanied by weight change, marked tiredness, or other new symptoms.
