What should you know about thermal hives?

Short answer

Thermal hives are itchy raised welts triggered by heat, sweating, exercise, hot water, or a rapid rise in body temperature.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common triggers
Heat, sweating, exercise, and hot water
Typical sensation
Itching, stinging, or tingling

What thermal hives can look and feel like

The rash may consist of small red or skin-coloured bumps, larger welts, or a flush that settles as the skin cools. It can affect areas that become hot and may come and go quickly. Itching, stinging, or tingling is common. Swelling around the eyes, lips, hands, or throat is a different level of concern and can occur with hives; see information about Angioedema for context.

Heat-triggered hives fall within the broader group of Hives, but not every heat rash is urticaria. A clinician may ask what happened before the rash, how long each welt lasted, and whether pressure, food, medicines, or infection also trigger episodes.

What you can do during an episode

Move to a cooler place, loosen tight clothing, and use a cool—not icy—compress. Avoid hot showers, saunas, strenuous activity, and rubbing the skin until symptoms settle. Keep a note of triggers, timing, affected areas, and any swelling. A pharmacist or clinician can advise whether an oral antihistamine is suitable; treatment choices depend on your health and other medicines.

Do not start oral corticosteroids for recurring hives without medical advice. They may be considered in selected situations, but the cause and severity need assessment.

When to seek care

Get emergency help immediately for trouble breathing, throat tightness, faintness, confusion, or rapidly increasing swelling of the tongue or lips. Seek prompt medical review if episodes keep returning, interfere with sleep or activity, last longer than expected, or leave bruising or pain. A new widespread rash after a medicine also needs urgent advice.

Questions for your doctor

Could this be heat-triggered urticaria rather than another rash? Which triggers should I avoid, and is an antihistamine appropriate for me? What symptoms would mean I need emergency help?

Questions people ask

Are thermal hives the same as heat rash?

No. Thermal hives are transient raised welts caused by a skin reaction; heat rash usually reflects blocked sweat ducts and has a different appearance.

Can cooling help thermal hives?

Cooling the body and avoiding the trigger may ease symptoms, but recurrent episodes should be assessed so suitable treatment can be discussed.
