Tetanus shot after a rusty nail: what to know

Tetanus is an infection caused by bacteria that can enter through broken skin. A nail puncture can push bacteria into deeper tissue, especially when the wound is dirty, deep or difficult to clean. Rust itself does not cause tetanus; the concern is contamination and the protection provided by your vaccination history.

A tetanus vaccine helps the immune system recognise the toxin made by these bacteria. Protection can become less certain over time, and the advice after an injury depends on the wound and the dates and type of previous vaccines. A clinician may also consider a different preventive treatment for some high-risk wounds when vaccination history is incomplete or unknown.

Wash your hands, then rinse the puncture under clean running water and gently clean around it with soap. Remove visible dirt only if it is easy to reach. Do not probe deeply, pour harsh chemicals into the wound or try to close a contaminated puncture yourself. Cover it with a clean dressing and seek medical advice promptly.

Take any vaccination record you have, or tell the clinician when your last tetanus-containing vaccine was given. Mention whether the nail went through footwear, whether it broke off, and whether you can move and feel the affected area normally. A healthcare professional will decide whether a booster, wound care, antibiotics or another treatment is appropriate.

Arrange urgent assessment if the nail is still embedded, the wound is deep or heavily contaminated, bleeding will not stop with firm pressure, or you have reduced movement, numbness or severe pain. Prompt care is also needed if redness, warmth, swelling, drainage or fever develops, as these can signal a wound infection.

Seek emergency help for trouble swallowing or breathing, severe muscle stiffness or spasms, confusion, or rapidly worsening illness. These symptoms need immediate assessment and should not be monitored at home while waiting for a routine appointment.

Ask: Does this wound need a tetanus booster based on my vaccine record? Is the puncture deep enough to need professional cleaning? Should I watch for signs of infection, and when should the dressing be changed? If you are pregnant, immunocompromised or allergic to a previous vaccine, mention this before treatment is chosen.
