Tension headache and tiredness: what to know

Short answer
Tiredness can accompany a tension-type headache, but persistent fatigue or a changing headache pattern deserves a medical review.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why tiredness may occur
Pain can make sleep less restorative and concentration harder, while long screen sessions, jaw clenching, stress, missed meals, or dehydration may add to both headache and fatigue. Tiredness may also reflect poor sleep, anaemia, thyroid problems, infection, mood changes, or a medicine effect, so it should not automatically be attributed to the headache.
Consider the pattern: fatigue that improves after sleep and hydration may differ from exhaustion that persists, steadily worsens, or comes with breathlessness, palpitations, weight change, fever, or low mood. A symptom diary can reveal links with sleep, meals, work, and headache days.
Helpful measures
Keep a regular sleep schedule, eat at consistent times, drink enough fluid, and take short movement or eye breaks during screen work. Gentle neck and shoulder movement can reduce muscle strain. Limit caffeine late in the day and avoid using pain medicines more often than the label or clinician recommends.
Arrange a GP consultation when tiredness lasts, interferes with work or driving, or occurs with frequent headaches. Bring a list of medicines and a record of sleep, food, fluids, stress, and symptoms.
Warning signs
Seek urgent help for a sudden severe headache, fainting, confusion, new weakness or numbness, trouble speaking, seizure, fever with a stiff neck, chest pain, severe breathlessness, or a headache after injury. Do not drive if tiredness makes you unsafe.
What to ask
Ask what might explain the fatigue, whether your sleep or medicines need review, whether blood tests are appropriate, and which headache features should prompt urgent care. Ask how to pace activity while symptoms settle.
