Tension headache when lying down: what to know

Short answer
A tension headache noticed while lying down may relate to neck position, but a new positional or worsening headache should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why lying down may change symptoms
Pillow height, neck angle, jaw clenching, or pressure on tight shoulder muscles can make tension-related discomfort more noticeable in bed. Some people only become aware of a mild headache when the day’s distractions stop. However, a headache that reliably worsens when lying flat, wakes you repeatedly, or is accompanied by vomiting or visual symptoms has a different level of concern and should not simply be assumed to be tension-type headache.
Try a careful routine
Check that your pillow keeps your neck in a neutral, comfortable position rather than sharply bent. Relax your jaw, avoid prolonged phone use in bed, and consider gentle shoulder movement before resting. Keep a record comparing symptoms when upright, reclining, and lying flat, along with sleep quality, congestion, nausea, and vision changes. Do not keep increasing pain medicine without guidance; arrange a GP consultation if the positional pattern is new, persistent, or progressing.
Do not wait with these symptoms
Seek urgent medical help for a sudden severe headache, new confusion, weakness, numbness, speech difficulty, fainting, seizure, repeated vomiting, fever with a stiff neck, or new vision loss. Prompt review is needed if lying flat consistently makes the headache markedly worse, if it wakes you from sleep, or if it follows a head injury. A changing pattern should be checked even when the pain is mild.
Questions to bring
Ask whether pillow position, jaw tension, sinus symptoms, migraine, or another cause fits your pattern, and whether an examination is needed. Explain how quickly the pain changes with posture and whether it interrupts sleep.

Questions people ask
Can a pillow trigger a tension headache?
An unsuitable pillow can strain the neck and shoulders and may make tension-related head pain more noticeable.
Why should a headache worse when lying down be checked?
A clear positional change can have causes beyond muscle tension, especially when it is new, progressive, or paired with vomiting or visual symptoms.
