Short answer

Tension headache pain is often felt across the forehead, temples or back of the head, with tightness that may involve the neck and jaw.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Where tension headache can hurt

The discomfort is commonly pressing or aching and may affect both sides, although the exact pattern varies. It can begin around the forehead or temples and extend over the scalp, behind the head, into the neck or across the shoulders. Jaw clenching, prolonged sitting, stress, tiredness and close screen work may add muscle tension.

A headache that is abrupt and extreme, follows a head injury, steadily changes, or comes with fever, weakness, confusion, speech difficulty or vision loss needs a different level of attention. The location of pain does not establish the diagnosis by itself.

Ways to ease it

Take a break from close work, move the neck and shoulders gently, unclench the jaw and try warmth over tense muscles. Regular meals, fluids and a consistent sleep routine may help prevent episodes. If you take an over-the-counter pain reliever, follow the label and seek advice if you need it often.

Record the area affected, duration, severity, triggers and associated symptoms. A GP can assess a new, frequent or disruptive pattern and discuss suitable treatment.

When to seek care

Seek emergency care for a sudden severe headache, new weakness or numbness, confusion, collapse, trouble speaking, a stiff neck with fever, or new vision loss. Arrange prompt medical advice for headaches after injury, repeated vomiting, persistent one-sided pain or a pattern that is worsening.

Questions for your doctor

Ask whether the location and quality fit a tension-type headache, what triggers may be relevant, and how often it is safe to use pain medicine. Share your headache diary and mention jaw clenching, neck strain and visual symptoms.

Questions people ask

Can tension headache spread into the neck?

Yes. Tightness in the scalp, jaw, neck and shoulders can accompany or extend the discomfort. Persistent neck pain or neurological symptoms need review.

Does the location prove it is a tension headache?

No. Location is only one part of the pattern; timing, quality, associated symptoms and examination also matter.
