TCA peel for ice pick scars: what to know

A clinician places trichloroacetic acid inside selected narrow acne scars. The controlled chemical injury encourages new skin repair within the depression. It is designed for sharply edged, deep scars rather than every type of acne scarring. Results develop gradually as treated spots heal, and more than one visit may be considered.

Temporary redness, crusting, swelling, tenderness, darkening or lightening can occur. Infection and a more noticeable scar are uncommon but possible concerns. Active acne, irritated skin, a tendency toward raised scars, or a history of pigment changes may alter the plan.

Have the scars examined in good light and ask whether punch treatment, microneedling, laser treatment or TCA CROSS better matches their shape. Tell the clinician about medicines, cold-sore history, eczema, recent tanning and previous pigment changes. Follow instructions about pausing irritating products.

After treatment, keep the area clean, do not pick crusts, use the recommended bland ointment and protect healed skin from sunlight. Avoid acids, scrubs, retinoids and makeup until the clinician says the surface has recovered.

Contact the treating clinic promptly if pain, warmth, swelling, discharge or redness is increasing instead of settling, or if blisters or fever appear. Seek urgent eye care for vision change or severe swelling near the eye. A review is also useful when pigment change or delayed healing persists.
