Can syphilis spread through saliva?

Syphilis is caused by bacteria that pass through direct contact with a sore or rash, often during oral, vaginal or anal sex. A sore may be inside the mouth, on the lips or in the genital or anal area, and it can be easy to miss because it may not hurt. Kissing without contact with an infectious sore is not considered a typical way to acquire syphilis. Sharing cups, food or utensils does not usually spread it.

A person with an oral syphilis sore can pass the infection during oral contact. The appearance of a sore cannot confirm its cause, so testing is the practical way to clarify risk.

Arrange an STI assessment if your mouth or genitals touched a partner's sore, if a partner has been diagnosed, or if you develop a mouth sore, genital sore, rash or swollen glands. Tell the clinician when the contact happened and whether it involved oral sex. Testing may involve a blood sample and, when a sore is present, an examination or sample from it.

Until you receive advice, avoid sexual contact with areas that have sores and tell recent partners if a clinician confirms syphilis. Syphilis can be treated with prescribed antibiotics, but the medicine and follow-up depend on the stage and your health.

Seek prompt medical care for a new mouth or genital sore, a widespread rash, eye pain or vision changes, hearing changes, severe headache, confusion or weakness. If you are pregnant and may have been exposed, contact your maternity or sexual-health team promptly because timely assessment helps protect both you and the pregnancy.

Ask which tests fit the timing of the exposure, whether a repeat test may be needed, whether a current sore should be sampled, and when sexual contact can safely resume. You can also ask how partners should be notified and whether treatment is needed before results return.
