What a swollen, hard tummy can mean

Abdominal swelling may feel tight, heavy, full or firm. Gas, constipation, food intolerance and changes in the abdominal wall are common possibilities. Fluid collecting inside the abdomen is called ascites; it can cause increasing waist size, early fullness when eating and a stretched feeling. Liver disease, liver cancer, ovarian cancer and myelofibrosis are among conditions that can be associated with abdominal swelling, but a swollen tummy does not identify any one diagnosis.

A clinician may ask about bowel movements, appetite, pain, weight change, menstrual or pregnancy possibilities, alcohol use and medicines. Examination and selected tests can help distinguish gas or constipation from fluid, an enlarged organ, a mass or another cause.

Arrange a medical review if the swelling is new, persistent, progressively firmer or interfering with eating, sleep or movement. Record its onset, changes in size, bowel pattern, pain and breathlessness. Continue usual prescribed medicines unless a clinician advises otherwise, and avoid self-treating persistent swelling with laxatives or fluid tablets without guidance. A healthcare professional can decide whether examination, blood tests or abdominal imaging is appropriate.

Seek urgent help for severe or rapidly increasing pain, repeated vomiting, a very tender or rigid abdomen, inability to pass stool or gas with worsening swelling, black or bloody stool, fever with marked illness, fainting or new breathing difficulty. These features can signal obstruction, bleeding, infection or pressure affecting breathing.

Ask what may be causing the firmness, whether fluid is present, which examination or scan is needed, and how to manage discomfort or early fullness safely. Ask which symptoms should prompt urgent review and when the team wants to reassess the swelling.
