Short answer

A swollen ankle after a fall may be a sprain or fracture; protect it, limit weight-bearing, and seek assessment when pain or function is significantly affected.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the injury may represent

A sprain stretches or tears ankle ligaments and can cause swelling, bruising, and tenderness around the joint. An Ankle Fracture may cause marked pain, swelling, deformity, or inability to take steps, but a person may still move an injured ankle, so movement alone does not rule it out. Ankle Osteoarthritis and Gout can also cause swelling, yet a clear fall makes an injury the first concern. An examination may be needed to decide whether imaging is appropriate.

First steps at home

Stop activities that increase pain and support the ankle in a comfortable position. Apply a cold pack wrapped in cloth for short periods, elevate the foot, and use a light support only if it does not cause numbness or colour change. Do not force the ankle straight or try to walk through severe pain. If you can do so safely, note where the tenderness is and whether the toes feel normal.

When to seek care

Get urgent assessment for an ankle that looks crooked, a wound over the painful area, severe or rapidly increasing swelling, numb or cold toes, or inability to bear weight. Seek prompt review if pain remains substantial, bruising spreads, or walking is still difficult after initial rest. Emergency help is appropriate for fainting or another serious injury from the fall.

Questions for your doctor

Ask whether the injury needs an X-ray, how much weight you may place on the foot, and whether a brace or crutches would help. Explain how you landed, where pain is worst, and whether you heard a crack or felt the ankle give way.

Questions people ask

Can a sprain swell as much as a fracture?

Yes. Swelling alone cannot distinguish them, so difficulty bearing weight or focal bone tenderness should be assessed.

Should I keep walking on the ankle?

Only if walking is comfortable and safe; use support and arrange assessment when weight-bearing is painful.
