Swelling with pre-eclampsia

Swelling in the feet can occur in pregnancy and does not prove pre-eclampsia. The condition is assessed using blood pressure, urine testing, symptoms, and sometimes blood tests or checks on the baby. Swelling that is sudden, marked, or also affects the face and hands is more concerning, particularly when it appears with headache or visual changes.

Pre-eclampsia can develop during pregnancy or after birth. It may affect organs such as the liver, kidneys, brain, or placenta, which is why a maternity team may recommend observation even when swelling is the symptom you noticed first.

Follow the assessment plan given by your maternity team and take prescribed blood-pressure medicine as instructed. Rest while arranging care, keep a record of symptoms, and bring your medication list. Do not self-treat with water tablets, leftover medicine, or a strict diet. Attend recommended monitoring appointments, which may include checks of your blood pressure, urine, blood, and baby’s wellbeing.

Contact your maternity unit urgently for a severe or persistent headache, flashing lights or blurred vision, pain under the ribs, repeated vomiting, sudden face or hand swelling, or a clear reduction in the baby’s movements. Call emergency services for seizure, confusion, severe breathlessness, chest pain, fainting, or collapse. After birth, the same warning symptoms still need immediate attention.

Ask what your blood-pressure reading and urine result mean, how often you need monitoring, and which symptoms require hospital care. Discuss how the condition may affect timing or mode of birth, including induction or caesarean section, and what follow-up is needed after delivery.
