Short answer

Swelling in the third trimester is often caused by fluid retention and pressure from the growing womb, but sudden or marked swelling needs prompt maternity assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common pattern
Gradual swelling in both feet or ankles, often worse later in the day
Assessment trigger
Sudden swelling of the face or hands, especially with headache or visual symptoms

What late-pregnancy swelling can mean

Extra fluid commonly collects in the feet and ankles after standing, walking, or later in the day. The womb can also slow blood return from the legs, making shoes or rings feel tighter. Mild swelling that develops gradually and affects both sides may fit this pattern.

Swelling of the face or hands, especially when it appears quickly or comes with a severe headache, vision changes, pain below the ribs, nausea, or feeling very unwell, can be a presentation of pre-eclampsia. Blood pressure and urine checks are needed to assess this; symptoms alone cannot confirm the cause.

What you can do today

Rest with your feet raised, change position often, and avoid standing still for long periods. Comfortable shoes and loose socks can reduce pressure. Gentle ankle movements and short walks may support circulation if your maternity team has not restricted activity.

Drink normally, eat regular balanced meals, and do not take water tablets or herbal remedies for swelling unless prescribed. Remove rings if fingers begin to swell. Note when the swelling started, where it is located, and whether it changes overnight so you can describe the pattern clearly.

When to seek care

Contact your maternity unit promptly for sudden swelling of the face or hands, a severe or persistent headache, blurred vision or flashing lights, pain beneath the ribs, vomiting, or feeling unusually unwell. These symptoms can occur with pre-eclampsia, a condition that needs assessment.

Seek urgent help for chest pain, difficulty breathing, fainting, or one leg that becomes painful, hot, red, or much more swollen than the other. Take your pregnancy records and medication list if you attend an assessment.

Questions for your maternity team

Ask: Could my swelling pattern be related to my blood pressure? Do I need a blood-pressure reading or urine test today? Which changes should make me call overnight? Is movement, compression wear, or a particular sleeping position suitable for me?

Questions people ask

No. Gradual swelling in both ankles is common, but a sudden change or additional warning symptoms should be assessed promptly.

No. Drink normally unless your maternity clinician gives different advice, and avoid medicines for swelling without guidance.
