Swelling in the ankle area: what to know

Short answer
Swelling in the ankle area may follow a sprain, fracture, arthritis, gout, or fluid buildup, so pain, timing, and movement matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Possible explanations
Swelling after a twist can indicate an ankle sprain, while swelling after a fall or direct impact may accompany an ankle fracture. Ankle osteoarthritis may cause stiffness, aching, and swelling that builds with use. Gout commonly presents with sudden, intense joint pain, warmth, redness, and swelling. Swelling in both ankles or swelling that changes through the day can have causes beyond the joint. Notice whether the area is hot, whether you can bear weight, and whether the swelling is limited to one side.
What to do at home
Reduce walking and avoid uneven ground while the ankle is painful. Elevate the leg during rest and use a wrapped cool pack after a minor injury. Wear supportive footwear and avoid tight straps over the swollen area. Gentle ankle movement can reduce stiffness if it is comfortable, but do not force the joint or return to sport while weight-bearing remains painful. A clinician should assess swelling that persists, repeatedly returns, or has no clear trigger.
When care is urgent
Seek urgent assessment if the ankle looks misshapen, pain is severe after an injury, you cannot take steps, or the foot becomes numb, pale, blue, or cold. Get prompt help for a hot, red, very painful joint with fever or feeling unwell. One-sided swelling with calf pain or breathlessness also needs urgent medical attention. Otherwise, arrange a review when symptoms are not improving or walking remains limited.
Questions for your doctor
Ask whether the swelling is inside the joint or in nearby tissue, whether an X-ray or other assessment is needed, and which level of walking is safe. Ask how to restart strengthening and balance work once pain settles.

Questions people ask
What can cause a swollen ankle without a clear injury?
Joint inflammation, osteoarthritis, gout, fluid buildup, and problems in nearby tissues can all cause swelling without a memorable injury.
Should I walk on a swollen ankle?
Only walk as much as comfort allows, using support if advised. Increasing pain or inability to bear weight warrants assessment.
