What should you know about swelling behind knee after running?

Short answer

Swelling behind the knee after running may reflect excess joint fluid, a fluid-filled cyst, muscle or tendon strain, or an injury that needs assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What it may mean

A feeling of fullness behind the knee can occur with a Baker's cyst, often when fluid from the knee joint collects at the back. Training changes, hills, or inadequate recovery may irritate muscles or tendons around the knee. A meniscus tear or ACL tear may be considered when swelling follows a twist and the knee catches, gives way, or feels unstable. Gradual stiffness can occur with knee osteoarthritis, while a hot, red, suddenly painful knee can occur with gout or another inflammatory problem.

The timing matters: swelling during a run, soon afterward, or the next day gives different clues. Record distance, terrain, recent training changes, pain location, locking, and whether the calf is also swollen.

What to do after the run

Stop or shorten runs that trigger swelling and switch temporarily to comfortable, low-impact movement if it does not worsen symptoms. Use a wrapped cold pack for discomfort, elevate the leg when resting, and avoid deep squats and forceful stretching while the knee is reactive. Gentle bending and straightening can maintain mobility. Arrange a clinical assessment if swelling keeps returning, limits normal walking, or persists despite reducing the provoking load; joint aspiration is one treatment a clinician may discuss when fluid is substantial.

When to seek care

Seek urgent help for sudden calf swelling or pain with breathlessness or chest pain, a hot red knee with fever, inability to bear weight, a locked or deformed knee, or a cold or numb foot. Prompt review is warranted after a twist with immediate swelling, repeated giving way, or worsening pain behind the knee.

Questions for your doctor

Ask whether the swelling is inside the joint, behind the knee, or in a muscle or tendon. Discuss whether imaging is needed, how to adjust training, which strengthening exercises fit your symptoms, and what return-to-running signs to use.

Questions people ask

Is swelling behind the knee after running always a Baker's cyst?

No. A Baker's cyst is one possibility; overuse, tendon or muscle irritation, joint inflammation, and internal knee injury can produce similar symptoms.

When can I run again?

Return when walking and knee movement are comfortable, swelling is settling, and a gradual increase does not bring symptoms back.
