Twisting can stretch or tear ankle ligaments, causing an ankle sprain with tenderness, bruising, and difficulty walking. Pain directly over a bone, marked tenderness, or inability to take a few steps raises concern for an ankle fracture. Less commonly, gout or ankle osteoarthritis can contribute to swelling, especially when there was no clear injury.
The amount of swelling alone cannot tell you how severe the injury is. A clinician may examine the ankle, check circulation and sensation, and arrange imaging when the injury pattern suggests a fracture.
What to do today
Protect the ankle from another twist and reduce weight-bearing if walking hurts. Elevate it, use a wrapped cold pack for short periods, and consider a comfortable support if a clinician has advised one. Keep the toes and ankle moving gently within comfort unless you were told to immobilise it. Avoid heat, massage, and sport while swelling is increasing.
Arrange review if walking remains difficult, pain is not easing, or swelling is not gradually improving. Record when the twist happened and whether you heard a snap.

Seek urgent care for a visibly deformed ankle, an open wound, numb or pale toes, severe worsening pain, or an inability to bear weight. Get prompt help for a hot red ankle with fever or sudden calf swelling with breathlessness or chest pain.
