What should you know about suture for cervical cerclage?

The cervix is the lower, narrow part of the uterus that opens into the vagina. During a cerclage, a clinician places a band of suture material around the cervix and tightens it to provide support. The stitch does not hold the baby itself; it supports the cervical tissue. A cerclage may be considered when the cervix has shortened or opened early, or when pregnancy history suggests a higher risk of cervical opening. It cannot address every cause of early labour, so the maternity team chooses it only after considering examination findings, pregnancy stage, symptoms, and previous history.

The suture may be placed through the vagina or, in selected situations, by an abdominal approach. The procedure and anaesthetic plan depend on the type of cerclage and your health. The stitch is generally removed before labour or when the care team advises; an abdominal cerclage may require a different birth plan. Your clinician should explain which type you have and how it affects delivery.

Follow the discharge instructions about rest, activity, bathing, sex, medicines, and follow-up. Mild spotting, pelvic pressure, or cramping can occur after the procedure, but your own instructions take priority because recovery differs. Keep scheduled maternity appointments so the cervix, pregnancy, and suture can be assessed. Do not pull, check, or try to remove the stitch yourself. Before taking a new medicine or returning to strenuous activity, ask the clinician who placed the cerclage what is appropriate for you.

Contact your maternity unit promptly for regular or worsening contractions, persistent lower abdominal or back pain, vaginal bleeding, leaking fluid, fever, chills, foul-smelling discharge, or new pressure in the pelvis. These symptoms can signal a problem that needs assessment, including labour, infection, or rupture of the membranes. Seek urgent help if bleeding is heavy, fluid is continuously leaking, or you feel seriously unwell. Do not wait for a routine appointment when symptoms are changing.

Ask: Which type of cerclage was recommended, and why? What symptoms should prompt a same-day call? Which activities should I pause, and for how long? When will the suture be reviewed or removed? Could the stitch change my birth plan? What should I do if contractions, bleeding, or fluid leakage starts? Writing down the answers can help everyone involved in your pregnancy care follow the same plan.
