Subcutaneous Morphea: What to Know

The affected area may feel firm, bound down or less flexible than nearby skin. Surface colour can change, but deeper disease may be less obvious at first. A lesion close to a joint can interfere with stretching; involvement beneath a limb or the trunk may produce a deep groove or an uneven contour.

Morphea is different from systemic scleroderma: it is localised to areas of skin and underlying tissue, although the pattern and depth still need specialist assessment. Examination may include checking skin texture, movement and nearby structures.

Arrange dermatology review for a firm, changing or tethered patch, especially if it crosses a joint or follows a line on a limb. Note its colour, texture, tenderness and effect on movement. Do not scrape or aggressively massage the area.

Treatment is individualised. Topical Calcineurin Inhibitors may be considered for selected superficial inflammation, while Narrowband UVB Phototherapy is one option used for some skin-limited patterns. Deeper or active disease may require a different plan, so treatment should follow examination.

Seek review if the patch expands, becomes more indurated, causes pain, tightness or weakness, or limits a child's or adult's movement. Contact a clinician promptly for rapid progression, ulceration, marked swelling, or symptoms affecting function near the eyes, mouth or a joint.

Ask whether the disease appears active or inactive, how depth will be assessed, what movement exercises are safe, which treatment fits the site, and how progress will be monitored.
