Strep throat and a face rash

Strep throat can present with a fine, rough-feeling rash that may begin on the trunk and spread, sometimes involving the face. Fever, a sore throat, inflamed tonsils, and swollen neck glands may occur at the same time. A rash limited to the face can also result from irritation, allergy, another infection, or a medicine reaction, so the rash alone cannot confirm the cause.

Note when the rash began, where it started, whether it is itchy or painful, and whether it blanches when pressed. Take a clear photograph if it changes quickly, but do not delay care when your child or you seem seriously unwell.

Contact a GP for assessment when a new rash appears with a sore throat or fever. Keep the skin cool, avoid new creams or fragranced products, and encourage fluids. Do not start antibiotics or use someone else’s medicine based only on the rash. If a rash began soon after a prescribed medicine, contact the prescriber promptly for advice about the next dose.

Get urgent help if the rash is purple or bruised-looking, rapidly spreading, blistering, or accompanied by swelling of the lips or tongue, wheezing, breathing difficulty, faintness, severe headache, stiff neck, or unusual drowsiness. Urgent assessment is also needed when swallowing fluids becomes difficult.

Ask whether the rash pattern fits strep throat, whether a throat test is appropriate, how to protect irritated skin, and which changes should prompt immediate review.
