Stelara dose escalation in ulcerative colitis: what to know

Stelara, also called ustekinumab, is a biologic medicine that targets immune signals involved in inflammation. Escalation is a treatment decision made after reviewing symptoms, bowel inflammation, blood or stool tests, and your treatment history. It is not something to change by yourself.

Your specialist may consider an adjusted schedule if diarrhoea, rectal bleeding, urgency, or abdominal pain continues, or if symptoms return before the next injection. They may also check whether symptoms come from active ulcerative colitis, an infection, or another problem before changing treatment.

A dose adjustment does not replace monitoring. Your clinician may assess bowel symptoms, inflammation markers, stool testing, and sometimes endoscopy. The aim is to control inflammation while keeping treatment safe and practical. The plan may later be continued, changed, or stepped back according to your response.

Keep a record of bowel frequency, urgency, bleeding, pain, fever, weight change, injection dates, and any new medicine. Contact your gastroenterology team before taking an early dose or changing the schedule. Ask whether testing is needed to distinguish a flare from infection or a medicine-related problem.

Continue injections exactly as prescribed unless your treating team tells you otherwise. Tell them about infections, recent antibiotics, vaccinations, allergies, and any planned procedure. Do not stop Stelara suddenly without advice, because your symptoms and inflammation may return.

Seek urgent medical care for severe or worsening abdominal pain, a swollen abdomen, repeated vomiting, heavy rectal bleeding, fainting, confusion, or signs of dehydration such as very little urine. A high fever or feeling suddenly very unwell also needs prompt assessment, especially while taking a biologic medicine.
