Wrist sprain or strain: what to know

How the structures involved in Ligament Sprain differ from normal

Short answer

A wrist sprain affects ligaments, while a strain affects muscle or tendon, and either can cause pain with gripping or movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Sprain
Ligament injury around the wrist
Strain
Muscle or tendon overload
Protective habit
Avoid painful gripping while maintaining gentle finger motion

What it means

A fall onto the hand, a sudden twist, repetitive keyboard or tool use, or lifting can overload the wrist. Sprain pain may be near a joint ligament; strain discomfort may follow a tendon or muscle path. Swelling, bruising, stiffness, reduced grip, and pain when turning the palm are possible. A small fracture or cartilage injury can look similar, particularly after a fall, so severe tenderness or loss of function should not be self-diagnosed.

Practical first steps

Rest from painful gripping, pushing, and lifting while keeping the fingers moving gently. Support the wrist in a neutral position if advised, and use a wrapped cold pack after a recent injury. Do not make a tight support that causes tingling, colour change, or increasing swelling. [Splinting](/treatments/trauma-and-fracture-care/splinting/) may help protect selected injuries. Once pain settles, [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) can restore motion and load tolerance. [Kinesiology Taping](/treatments/physiotherapy-and-rehabilitation/kinesiology-taping/) is an optional supportive technique.

When to seek care

Seek same-day assessment after a fall if the wrist is deformed, markedly swollen, very tender over a bone, or difficult to move. Urgent help is needed for a pale or blue hand, new numbness, or severe escalating pain. Arrange review when symptoms persist, grip remains weak, or pain returns with ordinary activity; an examination may distinguish a sprain from a fracture or tendon problem.

Questions for your doctor

Does the examination suggest a sprain, strain, fracture, or tendon injury? Should I use a splint, and for which activities? When should I start exercises and gradually reload the wrist?

Questions people ask

Can I exercise with a wrist sprain or strain?

Avoid movements that reproduce sharp pain; gentle range-of-motion work may be suitable when advised.

Why might a wrist injury need an X-ray?

A fracture can share the swelling and tenderness of a sprain, especially after a fall, so examination may warrant imaging.
