Spotting While Having Sex: Causes and Next Steps

Spotting can occur when intercourse causes minor friction to the vaginal wall or cervix. Dryness, hormonal changes, or a recent change in contraception can make this more likely. Infections such as chlamydia may inflame the cervix, sometimes without obvious symptoms. Polyps, fibroids, and changes in the cervix or womb can also contribute to bleeding. Cervical cancer is a possible cause of abnormal bleeding, but bleeding during sex does not establish that cancer is present.

Pay attention to whether the bleeding is only on tissue or continues onto a pad, whether it occurs after every encounter, and whether it comes with pain, discharge, odour, fever, or urinary changes. This pattern helps guide an assessment.

Pause sex when spotting starts and use a pad or liner to monitor it. Avoid douching or inserting products while the cause is uncertain. If symptoms settle, lubrication and slower, gentler activity may reduce friction, but repeated spotting should still be checked. Arrange a clinician visit if the bleeding happens again, occurs after a new partner, or is accompanied by pelvic pain, discharge, or urinary burning. Mention pregnancy possibility and any hormonal contraception.

Get urgent medical care for heavy bleeding, severe or one-sided pelvic pain, faintness, unusual weakness, fever, or possible pregnancy with pain or worsening bleeding.
