What should you know about split cast for fracture?

A cast is split lengthwise when the limb may swell after a fracture or after the bone has been repositioned. The opening gives the rigid shell some room and may be secured with a bandage or straps. Plaster of Paris and fibreglass are different cast materials; your clinician chooses based on the injury and stage of care.

A split cast still needs to protect the fracture from unwanted movement. Do not cut, trim, wet or reshape it. The opening does not mean the bone is ready for unrestricted use, and it does not replace scheduled checks of alignment, circulation and skin.

Keep the limb raised above heart level when practical during early swelling. Keep the cast dry, avoid inserting objects inside it, and follow instructions about weight-bearing or using crutches. Wiggle exposed fingers or toes gently if advised, and inspect them for colour, warmth, sensation and movement. Attend review appointments because the cast may need adjustment, repair or replacement as swelling changes.

Get urgent help for severe or increasing pain, numbness, inability to move exposed fingers or toes, or digits that become cold, blue or very pale. Contact the fracture service if the cast becomes wet, cracked, loose, foul-smelling or causes a pressure point. A new fall or impact while in the cast also warrants assessment.

Ask how the split is secured, whether you may put weight through the limb, how to sleep comfortably, and when the cast will be changed or removed. Clarify which symptoms require the emergency department.
