What should you know about spinal cord compression myelopathy?

It can develop when a tumour affects the bones of the spine or grows near the spinal canal. Swelling, a collapsed vertebra, or displaced tissue can add to the pressure. Prompt treatment aims to protect nerve function and address the cause.

The spinal cord carries messages between the brain and much of the body. Compression may cause new or worsening back or neck pain, tingling, numbness, leg weakness, heaviness, trouble walking, or poor balance. The pattern depends on the level of the spine involved.

In cancer care, spinal cord compression is a possible complication when cancer involves the spine. Multiple myeloma can affect bone and may be relevant when clinicians are investigating a spinal problem. Symptoms need assessment because nerve changes can progress.

Contact your cancer team or an urgent medical service straight away if these symptoms are new, worsening, or appearing together. The clinical team may examine strength, sensation, reflexes, and walking, then arrange imaging of the spine to locate the pressure.

Treatment is chosen around the cause, the location, and your overall health. Medicines may be used to reduce swelling. Palliative radiotherapy can shrink or control tumour tissue causing pressure, while tumour removal surgery may be considered when stabilising the spine or relieving compression is appropriate.

Seek emergency help now for sudden weakness, difficulty standing or walking, numbness around the genitals or buttocks, or a new inability to pass urine or control urine or stool. These signs can indicate significant nerve involvement and should not wait for a routine appointment.

While arranging care, avoid driving yourself if weakness or altered sensation could make this unsafe. Take a current medicines list and tell the team about any cancer diagnosis, recent treatments, and the time symptoms began or changed.

Ask which part of the spine is affected, what the scan shows, and whether the spine is stable. You can also ask what treatment is being recommended, how quickly it needs to start, which symptoms should trigger emergency care, and who to contact outside clinic hours.
