Sore throat and syphilis: what to know

Syphilis is an infection passed through sexual contact, including oral sex. A mouth or throat infection may cause a sore or ulcer, swollen glands, rash, or no noticeable symptoms. These features overlap with common conditions such as the Common Cold, Influenza, Tonsillitis and Strep Throat, so symptom appearance cannot establish the cause.

Tell the clinician about recent sexual contact, oral exposure, sores, rash, fever, or a partner who has been diagnosed. The conversation is routine and confidential. Testing may involve a blood sample and, when a sore is present, a sample from that area. A clinician can also assess for other sexually transmitted infections.

Arrange a sexual-health or primary-care appointment if exposure is possible, a mouth or throat sore is unexplained, or a partner has syphilis. Avoid sexual contact, including oral sex, until you have received advice and any recommended treatment is complete. Do not self-treat with leftover antibiotics, because the correct medicine and follow-up depend on the diagnosis.

Use ordinary throat comfort measures such as fluids and soft foods. If you are worried about privacy, ask the service how results, partner notification and follow-up are handled before testing.

Seek urgent care for breathing difficulty, inability to swallow fluids, rapidly increasing mouth or neck swelling, confusion, severe headache, or a rapidly spreading rash with marked illness. Otherwise, book timely assessment rather than waiting for symptoms to identify the cause.

Ask which tests are recommended for oral exposure, whether testing for other infections is appropriate, when results are expected, how to protect partners, and when sexual contact can safely resume.
