Why one side may hurt
Pain may feel one-sided when inflammation is stronger around one tonsil or when a small scratch, ulcer, or trapped food affects a particular area. Tonsillitis and strep throat can cause localized discomfort, while a common cold, influenza, COVID-19, or glandular fever may begin unevenly before broader symptoms appear.
Swollen neck glands can make nearby movement uncomfortable. Reflux, allergies, and mouth dryness can irritate one area more than another depending on sleep position or nasal blockage. The location of pain does not, by itself, identify the cause.
What you can do
Sip fluids, choose soft foods, and use warm or cool drinks according to comfort. Avoid sharp foods, smoking, and strenuous voice use while the tissue settles. Look for a visible ulcer or swelling only with a mirror and good light; do not poke the area or try to remove anything embedded.
Note whether pain is linked to swallowing, jaw movement, meals, or lying down. A clinician can examine the tonsils, mouth, neck, and ears, which may reveal a source that is not visible at home.
When to seek care
Seek urgent help for trouble breathing, drooling, inability to swallow, a muffled voice, rapidly increasing swelling, or difficulty opening your mouth. Arrange prompt assessment when one-sided pain is severe, the uvula appears pushed to one side, you feel markedly unwell, or symptoms continue to worsen.
