What should you know about smoldering multiple myeloma?

Short answer
Smoldering multiple myeloma means abnormal plasma cells or related protein are present, but there are no signs of active organ damage from myeloma.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cell type involved
- Plasma cells in the bone marrow
- Main approach
- Planned monitoring for signs of active disease
What the diagnosis means
This finding is different from active multiple myeloma, which causes organ or tissue problems. It is also different from a harmless antibody change because the marrow findings are more pronounced. The distinction depends on blood and urine testing, bone marrow examination, and imaging interpreted together.
Some people remain stable for a long time; others develop active disease. Monitoring helps the care team notice changing blood markers, anemia, kidney problems, high calcium, or bone changes before deciding whether treatment is needed.
What to do next
Keep the follow-up plan arranged with a hematology team. Bring a list of medicines and tell the team about new bone pain, unusual tiredness, repeated infections, thirst, constipation, changes in urination, or unexplained weight change. Follow-up may include blood counts, kidney and calcium checks, protein measurements, urine testing, and repeat imaging when clinically appropriate.
Treatment is not automatically started when smoldering disease is found. If active myeloma develops, options can include targeted therapy, chemotherapy, radiotherapy, bone-strengthening therapy, cellular treatment, or transplant approaches, selected for the individual situation.
When to seek care promptly
Contact the oncology team promptly for new or worsening back or rib pain, weakness, numbness, difficulty walking, marked thirst, confusion, reduced urine, fever, or breathlessness. Sudden severe back pain with leg weakness or loss of bladder or bowel control needs emergency assessment because nerve or spinal pressure must be addressed quickly.
Questions for your doctor
Ask which test results support smoldering disease, what changes would indicate active myeloma, how often follow-up is needed, and which symptoms should trigger a same-day call. You can also ask whether imaging, a bone marrow review, or a specialist opinion would clarify your risk and monitoring plan.
Questions people ask
Is smoldering multiple myeloma the same as active myeloma?
No. Smoldering disease has abnormal plasma-cell findings without myeloma-related organ damage; active myeloma has evidence that the disease is affecting the body.
Will everyone with smoldering multiple myeloma need treatment?
Not necessarily. The hematology team uses test results and changes over time to decide whether observation remains suitable or treatment should begin.
