What should you know about smoldering mm?

Multiple myeloma is a cancer of plasma cells, which are immune cells found in the bone marrow. In smoldering myeloma, tests show a higher amount of abnormal plasma cells or an abnormal protein, but there are not yet the organ problems that define active disease. This is different from a harmless laboratory finding and different from active multiple myeloma. The distinction depends on blood and urine tests, bone-marrow findings, imaging, kidney function, blood counts, calcium, and evidence of bone damage. A specialist combines these results rather than relying on one result alone.

Keep scheduled haematology appointments even when you feel well. Monitoring commonly involves repeating blood or urine tests and reviewing symptoms; imaging or a bone-marrow examination may be recommended when results or symptoms change. Tell the care team about new pain, infections, tiredness, bruising, thirst, or changes in urine. Ask whether medicines, dental care, activity, and vaccination planning need adjustment. Treatment is not automatically the next step for every person with smoldering disease. If active myeloma develops, options may include chemotherapy, targeted therapy, stem-cell transplant, cellular therapy, radiotherapy, and bone-strengthening therapy, selected according to the individual situation.

Contact your haematology team promptly for new or worsening bone or back pain, unusual weakness, repeated infections, unexplained bruising, reduced urine, marked thirst, constipation, or confusion. Seek urgent help for sudden severe back pain with leg weakness or loss of bladder or bowel control, fever with feeling very unwell, or inability to keep fluids down. These symptoms need assessment for complications and do not by themselves prove progression to active myeloma.

Ask which findings support the smoldering classification, how often monitoring is needed, and which symptoms should trigger an earlier call. Discuss whether repeat imaging, genetic or molecular tests, or a clinical trial is suitable, and what would change the plan to active-myeloma treatment.
