Small Spotting During Pregnancy: What to Know

Spotting means a small amount of blood, often noticed on toilet paper or underwear. It is different from bleeding that soaks a pad or comes with clots. The timing, colour, amount, pregnancy stage, and accompanying symptoms help a clinician decide what assessment is needed.

Light spotting may happen when the cervix is more sensitive during pregnancy, including after sex or an examination. It can also occur with irritation or an infection affecting the vagina or cervix. Chlamydia is one infection that can cause bleeding between periods or after sex and needs testing and treatment.

Bleeding can also be linked to changes inside the uterus or to pregnancy-related problems. A symptom alone cannot identify the cause, and a small spot does not confirm or rule out a serious condition. A clinician may ask about pain, dizziness, fever, discharge, recent sex or examination, and previous pregnancy history before deciding whether you need an examination, ultrasound, or tests.

Contact your maternity team, obstetrician, midwife, or an urgent medical service and explain that you are pregnant and have noticed bleeding. Note when it began, whether it is pink, brown, or red, how much you see, and whether it changes. Also mention cramps, one-sided pain, shoulder pain, fever, faintness, unusual discharge, or pain when passing urine.

Use a pad if you need to monitor the amount; do not use a tampon. Follow the clinician's advice about sex, activity, and medicines until you have been assessed. If you know your blood group or have had pregnancy complications before, include that information when you call.

Seek urgent medical care now if bleeding becomes heavy, you pass clots or tissue, or you develop severe or worsening abdominal pain. Get immediate help for fainting, marked dizziness, shoulder-tip pain, fever, or feeling very unwell. These symptoms need prompt assessment because bleeding during pregnancy can sometimes signal a condition requiring urgent treatment.

Do not wait for a routine appointment if the bleeding is increasing or is accompanied by pain. If the spotting remains slight and you feel well, still contact your pregnancy care team promptly so they can advise you based on your stage of pregnancy and medical history.
