What should you know about small cafe au lait spots?

Cafe au lait spots are flat patches that range from light tan to coffee brown. They may be present from birth or become noticeable during childhood. Their edges can be smooth or uneven, and the colour may be more visible after sun exposure. They do not usually itch, hurt, or feel raised.

One or a small number can occur without another health problem. A pattern of numerous spots, especially with other skin findings, freckles in unusual areas, changes in vision, or concerns about growth and development, should be discussed with a doctor. A photograph can help track size and colour over time.

Do not scratch, bleach, or repeatedly exfoliate the area. Use sun protection so the contrast does not become more noticeable, and take an occasional clear photograph beside a ruler if the spot seems to be changing. A clinician can inspect the patch and decide whether any further evaluation is appropriate.

If the appearance bothers you, ask about pigment-targeting treatment. Picosecond laser treatment may be considered for selected spots, but results vary and pigment can return or change after treatment. Skin type, location, and the diagnosis should be assessed first.

Book a routine review if a spot becomes raised, bleeds, crusts, rapidly changes in colour or border, or is joined by many new patches. Arrange assessment for a spot with persistent itching or pain. Seek urgent care for severe swelling, blistering, or a rapidly spreading skin reaction.
