What may be happening

An attack can begin during travel, crowds, enclosed spaces, conflict, medical procedures or another feared situation. The body’s alarm response may cause racing or pounding heartbeats, chest tightness, fast breathing, shaking, sweating, dizziness, or numbness and tingling. The sensations can be frightening, but they do not by themselves establish panic disorder. A clinician considers the pattern, triggers, avoidance and other possible medical explanations.

During and between attacks

If possible, move to a safer, quieter place and sit with both feet supported. Breathe gently and slowly without forcing deep breaths; remind yourself that the surge will pass. Avoid driving until you feel fully well. Record the setting, symptoms, duration and what helped. A GP consultation or mental health assessment can help identify triggers and discuss talking therapies, coping practice or other treatment.

When to seek care

Call emergency services for new or severe chest pain, fainting, major breathing difficulty, weakness on one side, confusion or symptoms that do not settle. Seek prompt assessment when attacks are new, recurring, causing avoidance or affecting work and sleep. Urgent mental health support is needed if fear is accompanied by thoughts of self-harm or inability to stay safe.
