Sinus and tension headache: what to know

Short answer
Sinus pressure and tension-type headache can feel similar, but congestion, facial symptoms and muscle tightness help guide the next step.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Tension pattern
- Pressure with possible scalp, jaw, neck or shoulder tightness
- Sinus clues
- Nasal blockage, facial pressure or reduced smell
- Useful record
- Note timing, triggers, symptoms and medicines used
What the symptoms may mean
A tension-type headache often feels like steady pressure or a band around the head. Tightness in the scalp, jaw, neck or shoulders may accompany it, and prolonged screen use, stress, poor sleep or an awkward posture can contribute.
Sinus inflammation or congestion can cause pressure around the cheeks, forehead or eyes, with a blocked nose, reduced sense of smell or thick nasal discharge. Facial pressure alone does not establish sinusitis; migraine and tension headache can also be felt in the face.
What you can do
Rest in a quiet space, drink fluids and try a warm compress over the face or neck. Gentle neck and shoulder movement, regular screen breaks and unclenching the jaw may ease muscle-related pressure.
If nasal congestion is present, saline rinsing may help some people when performed with sterile or previously boiled and cooled water. Use pain medicine only as directed on its label, and avoid frequent self-treatment because repeated use can itself sustain headaches.
When to seek care
Seek urgent medical help for a sudden, unusually severe headache, new weakness or numbness, confusion, fainting, seizure, stiff neck with fever, or a new headache after a significant head injury.
Arrange a GP assessment if headaches keep returning, change pattern, affect daily activities, occur with vision changes or vomiting, or do not improve with sensible self-care. A GP can assess your nose, nerves, eyes, jaw and neck.
Questions for your doctor
Ask whether your pattern fits tension-type headache, migraine or a sinus condition; whether nasal treatment is appropriate; and which pain medicines are suitable with your other conditions or medicines.

Questions people ask
Can sinus congestion cause a headache?
Congestion and inflammation can create facial pressure, while headache disorders can produce similar sensations. The accompanying symptoms and examination help separate them.
When should recurring headaches be assessed?
Book a GP assessment when headaches recur, change, interfere with life or come with neurological, visual or persistent vomiting symptoms.
