Silent heartburn: symptoms without a burning feeling

Acid or stomach contents can move upwards and irritate the food pipe or throat. Instead of classic heartburn, a person may notice repeated throat clearing, hoarseness, a sour taste, cough, a lump sensation, or symptoms that are worse after meals or when lying down. Acid reflux is a common explanation, but gastritis, a hiatus hernia, peptic ulcer disease, medicines, and throat conditions can produce overlapping symptoms. The absence of burning does not identify the cause by itself.

Notice whether symptoms follow particular meals, late eating, bending, or lying down. Try smaller meals, remain upright after eating, and avoid foods or drinks that clearly trigger symptoms. Do not stop prescribed medicines without advice. A clinician can review your symptoms and decide whether acid reflux medication or another approach is suitable, especially if symptoms are frequent or affecting sleep, voice, or swallowing.

Get emergency help for chest pressure with breathlessness, sweating, faintness, or pain spreading to the arm or jaw, because these symptoms may signal a heart problem rather than reflux. Arrange prompt medical assessment for food sticking, painful swallowing, vomiting blood, black stools, repeated vomiting, unexplained weight change, or persistent symptoms that do not settle.

Ask whether your symptoms fit reflux, whether another condition needs checking, and how long to try the recommended treatment. Mention swallowing changes, nighttime symptoms, medicines, and any pattern related to meals.
