Short answer

Pain when moving the arm behind your back often reflects restricted rotation, tendon irritation or joint stiffness rather than a single diagnosis.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Reaching behind the back

Pain when moving the arm behind your back often reflects restricted rotation, tendon irritation or joint stiffness rather than a single diagnosis. This movement combines shoulder extension and inward rotation, so it can expose limitations that are not obvious when the arm rests by your side. Note whether the problem is pain, stiffness, weakness or a catching feeling.

What may be involved

Frozen Shoulder can present with difficulty reaching behind the back along with restriction in other directions. Rotator Cuff Tendinitis or Shoulder Impingement can present with pain when the arm is positioned behind the body. Shoulder Osteoarthritis may cause aching, stiffness and grinding. A Rotator Cuff Tear can present with weakness after an injury. Calcific Tendinitis may cause sudden, intense shoulder pain. Neck nerve irritation is more likely to add tingling, numbness or pain travelling down the arm.

Protecting movement

Avoid forcing the hand farther up the spine or using the painful arm to pull clothing into place. Try easier options, such as bringing garments around in front, while keeping the shoulder gently active within comfort. Do not hold a stretch that causes sharp pain or lingering worsening. A physiotherapist can assess rotation and show graded mobility work rather than relying on forceful stretching.

When to get help

Arrange an assessment if reaching, washing or dressing remains difficult, or if stiffness is progressively increasing. Seek prompt care after a fall with sudden weakness, deformity or inability to use the arm. Emergency help is needed for chest symptoms, breathing difficulty, faintness or a hand that becomes cold, pale or numb.

Questions for your appointment

Ask whether the limitation is caused by pain or true stiffness, which movements are safe to practise, and whether weakness suggests tendon injury. Ask how often to do mobility exercises and what change should prompt an earlier review.

Questions people ask

Does difficulty reaching behind my back mean frozen shoulder?

Not necessarily. Frozen Shoulder is one possible explanation; tendon, joint and neck problems can produce different versions of the same difficulty.

Should I stretch farther every day?

Use gradual, comfortable movement rather than force. Increasing pain or next-day worsening means the activity needs adjusting.
