What should you know about shoulder arthrocentesis?

A clinician cleans the skin, may numb the area, and guides a needle into the joint. Fluid can be sent for testing for blood, infection, crystals, or other signs of inflammation. The procedure is different from surgery: it is usually performed through a small skin entry, and the clinician may use imaging guidance when the joint is difficult to access.

Fluid in a joint can build up after injury or because of inflammation. Crystal-related conditions such as Gout or Pseudogout can affect joints, while aspiration is also used in some fluid-filled problems such as a Baker's Cyst or Knee Bursitis.

Tell the clinician about blood-thinning medicines, allergies, skin infection near the shoulder, and any fever or recent illness. Wear clothing that leaves the shoulder easy to reach. Afterward, follow the dressing instructions, keep the area clean, and avoid strenuous shoulder use until you are told it is suitable. Mild tenderness can occur; ask what pain relief is appropriate for you.

Contact the treating service promptly if the puncture site becomes increasingly red, hot, swollen, or painful, or if fluid continues to drain. Seek urgent care for fever with worsening shoulder pain, feeling faint, new arm numbness, or marked weakness. These changes need assessment rather than being assumed to be a normal recovery.

Ask why aspiration is recommended, whether the fluid will be tested, when results are expected, how long to protect the shoulder, and which symptoms should trigger a call.
