What should you know about shoulder arm pain when lifting your arm?

Lifting can narrow or load spaces around the shoulder tendons, making an irritated tendon or joint painful. The discomfort may spread into the upper arm rather than stay at the shoulder. Rotator cuff tendinitis, impingement, calcific tendinitis, a tear or a stiff shoulder are possible explanations, while neck nerve irritation can mimic shoulder pain.

Clues in the movement pattern
Pain that appears during overhead work may occur with rotator cuff tendinitis or shoulder impingement. Severe pain and weakness after a fall or sudden pull raises concern for a rotator cuff tear. Painful stiffness that restricts dressing or reaching in many directions can occur with frozen shoulder. Tingling, burning pain or weakness extending below the elbow points more toward cervical radiculopathy. Shoulder osteoarthritis can cause aching, stiffness and grinding. A pattern can guide an examination but cannot establish the cause alone.

Pause repeated overhead lifting and keep loads close to the body. Use gentle movements below the painful range, change sleeping positions and avoid forcing a stiff shoulder. Cold or warmth may feel helpful; protect the skin and stop if symptoms worsen. A clinician can assess strength and movement, then decide whether physiotherapy, medication or imaging fits your situation. Keep notes on the height that triggers pain and whether the neck changes it.
When to seek care
Seek urgent care after an injury if you cannot lift the arm, the shoulder looks deformed, or the hand becomes cold, pale, numb or weak. Emergency help is needed for arm pain with chest pressure, breathlessness, faintness, sweating or nausea. Arrange a review for worsening weakness, fever, persistent night pain or symptoms that are limiting ordinary tasks.
