Sharp stomach pain around ovulation: what to know

Ovulation can irritate tissues around an ovary and cause one-sided lower abdominal pain. It may feel sharp, cramp-like, or ache for a limited time and can recur on a similar part of the menstrual cycle. The timing alone does not confirm the cause. Pain higher in the abdomen, pain linked to meals, bowel changes, urinary symptoms, or a delayed period can point to another explanation. Conditions such as Gastritis, Gallstones, Diverticular Disease, Gastroenteritis, Food Poisoning, Coeliac Disease, or Crohn's Disease have different patterns and may need examination.

Rest, use a wrapped warm pack on the lower abdomen, and drink fluids if you are able. Record the side, start time, severity, cycle day, bleeding, bowel symptoms, urinary symptoms, and any chance of pregnancy. Do not ignore pain that repeatedly disrupts normal activities. A clinician can consider your cycle history and examine the abdomen; pregnancy testing or other tests may be appropriate depending on the pattern.

Seek urgent care for sudden severe pain, fainting, marked dizziness, a rigid or very tender abdomen, heavy bleeding, persistent vomiting, fever, or pain during a possible pregnancy. Prompt assessment is also sensible when pain steadily worsens, follows an injury, or does not settle as expected. These features can signal a problem that should not be attributed to ovulation without examination.

Ask whether the location and cycle timing fit ovulation pain, whether pregnancy testing is needed, and what symptoms would change the urgency. Mention contraception, last menstrual period, bleeding, discharge, bowel or urinary changes, and any previous abdominal conditions.
