Sex after menopause bleeding: what to know

Bleeding after sex after menopause should be checked, even when it is light, because thin or fragile tissue and other conditions can cause it. Lower oestrogen can make vaginal tissue drier, less elastic and easier to irritate. A small amount of blood may follow friction, but you cannot identify the cause from the amount alone.

Genitourinary changes around menopause may cause dryness, burning, soreness or tiny surface cracks during penetration. A pelvic organ prolapse, cervical or vaginal growth, inflammation, infection, or a thickened uterine lining can also cause bleeding. Bleeding after sex is a symptom that needs assessment; it does not mean that a serious condition is present, but it should not be dismissed as a normal part of ageing.

Tell the clinician whether your periods have stopped, whether bleeding happens without sex, and whether you have pain, discharge, urinary symptoms or a new lump or pressure sensation. These details guide the examination.

Book a GP or gynaecology appointment for any bleeding after menopause. Until then, avoid penetration if it causes pain or bleeding. A vaginal moisturiser used regularly may help dryness, while a suitable lubricant can reduce friction during sex; ask a clinician or pharmacist which product fits your health needs. Do not self-treat repeated bleeding without an examination. If you also have hot flushes, night sweats, mood changes or low sex drive, mention these because they may be part of the wider menopausal picture.

Seek urgent care for heavy or rapidly increasing bleeding, faintness, severe pelvic pain, fever, or feeling acutely unwell. Arrange prompt assessment if bleeding recurs, occurs without sex, or comes with watery or foul-smelling discharge, unexplained weight change, a pelvic mass sensation or worsening pain. A clinician may examine the vagina and cervix and decide whether further tests are needed.

Ask what could be causing the bleeding, whether vaginal dryness or prolapse is contributing, whether local treatment is suitable, and whether you need an examination or further testing. Share all medicines, hormone treatments, bleeding episodes and associated symptoms.
