What should you know about severe sore throat not strep?

A negative strep test makes that particular bacterial cause less likely, but it does not identify the reason for pain by itself. Common viral infections can cause intense throat inflammation, with cough, runny nose, hoarseness, tiredness or fever. Tonsillitis may be viral and can cause enlarged, tender tonsils. Glandular fever may cause profound tiredness and swollen glands. Acid reflux, allergic rhinitis and dry air can irritate the throat without producing a strep infection.

Drink enough to keep your urine pale, choose soft foods and avoid smoke, alcohol and very spicy foods if they worsen burning. Warm or cool drinks may provide temporary comfort. Rest your voice and use labelled pain relief only if it is safe for you. A pharmacist can check interactions and suggest suitable options. If reflux seems possible, avoid lying down soon after eating and record foods or times that trigger symptoms.

Contact a clinician if severe pain is not easing, symptoms are worsening, fever persists, or you cannot maintain fluids. Seek urgent help for trouble breathing, inability to swallow saliva, drooling, a muffled voice, neck swelling, severe one-sided pain or difficulty opening the mouth. These features can reflect airway risk or a deeper infection even when strep is not present.

Ask whether another examination or test is needed, whether the pattern fits a viral illness, tonsillitis, glandular fever, reflux or allergy, and which treatments are safe. Tell the clinician exactly when the strep test was done and describe fluid intake, fever, cough, rash and swollen glands.
