Severe Nappy Rash in a Newborn: What to Know

Nappy dermatitis develops when delicate skin remains exposed to urine, stool, friction or cleansing products. Redness may be concentrated on areas touching the nappy. A yeast rash can be bright red, involve skin folds and have small spots around its edge. Crusting, blisters, pus, marked tenderness or sores can signal infection or a different condition.

Newborn skin can deteriorate quickly, and a rash may be harder to judge when the baby is very young. The rash itself does not explain every change in behaviour, feeding or temperature, so those details matter during assessment.

Change nappies promptly, rinse with lukewarm water or use a fragrance-free cleanser, and pat rather than rub. Allow brief nappy-free air time when practical. Apply a simple protective barrier recommended for newborn skin; Emollient Therapy describes products that reduce water loss and friction.

Avoid talc, fragranced wipes, harsh antiseptics and home remedies. Do not use steroid, antibiotic or antifungal products on a newborn without advice. If a yeast infection is suspected, a clinician may recommend Topical Antifungal Treatment.

Contact a clinician promptly for a newborn with a severe or rapidly worsening rash, broken skin, blisters, pus, bleeding, spreading redness, or significant pain. Seek urgent help if the baby is unusually sleepy, feeding poorly, difficult to settle, or has a fever or seems unwell. Medical review is also needed when careful nappy care does not start to help.

Ask whether the pattern suggests irritation, yeast, bacterial infection or another diagnosis; which barrier product is suitable; and what changes in feeding, temperature or behaviour should prompt urgent help.
