What should you know about severe intertrigo?

Intertrigo is inflammation where two skin surfaces rub together and hold warmth or moisture. It can affect the groin, underarms, under the breasts, the abdomen or other folds. The skin may look red, raw, cracked or weepy and can sting, itch or smell. A skin yeast infection may develop on the damaged surface and can add small spots or a more active edge, but a similar rash can also result from irritation, allergy or bacterial infection. Severe appearance does not identify the cause by itself; examination helps guide treatment.

Gently wash the fold, rinse away cleanser and pat it completely dry. Use a cool hairdryer setting if patting is uncomfortable, and place clean, breathable fabric between surfaces to reduce rubbing. Change damp clothing promptly and avoid fragranced products, harsh scrubbing and tight garments. Do not seal a wet rash under thick ointment unless a clinician recommends it. A topical antifungal treatment may be prescribed when yeast is suspected. Recurrent sweating may lead a clinician to consider prescription antiperspirant, but it should not be applied to broken or freshly irritated skin without instructions. Weight, diabetes, mobility, heat and clothing can all influence recurrence and may be discussed privately.

Seek prompt care if redness spreads beyond the fold, pain becomes marked, the area is hot or swollen, pus develops, the skin turns dusky, or fever or unwellness appears. Arrange assessment when the rash is cracked, bleeding, foul-smelling, repeatedly returning or not improving with careful drying. Earlier review is sensible for people with diabetes, reduced immunity or difficulty caring for the affected fold.
