Short answer

Severe hiccups that persist, interrupt sleep or eating, or come with other symptoms need medical assessment to find the cause.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why hiccups may continue

A hiccup is an involuntary spasm of the diaphragm followed by sudden closure of the vocal cords. Eating quickly, swallowing air, fizzy drinks, alcohol, sudden temperature changes, or emotional excitement can trigger short-lived episodes. Persistent hiccups may be linked with irritation of the nerves controlling the diaphragm, stomach irritation, acid reflux, or gastritis. Less commonly, medicines or conditions affecting the chest, brain, or body chemistry can be involved.

The pattern matters: record when the hiccups started, what you had eaten or drunk, whether lying down worsens them, and any pain, vomiting, cough, or difficulty swallowing.

What you can try

Sit upright, take small sips of water, eat slowly, and avoid large meals, fizzy drinks, alcohol, and foods that clearly trigger symptoms. If reflux seems involved, avoid lying down soon after eating and discuss suitable treatment with a clinician. Do not use frightening or forceful tricks, and do not hold your breath for a prolonged period. Book a review when hiccups keep returning or interfere with sleep, nutrition, hydration, or daily activities.

When urgent care is needed

Seek urgent help if hiccups occur with severe chest or abdominal pain, breathing difficulty, confusion, weakness, repeated vomiting, inability to swallow fluids, or signs of dehydration. Emergency assessment is appropriate when breathing or alertness is affected.

Questions for your doctor

Ask what could be irritating your diaphragm, whether reflux or gastritis should be assessed, and whether a medicine review or treatment is appropriate.
