Severe Diaper Rash: What You Should Know

Nappy rash is inflammation in the covered area. Severe irritation may be bright red, raw, swollen, or painful, with small cracks, erosions, or spots beyond the main patch. Moisture, rubbing, urine, stool, and fragranced products can weaken the protective skin barrier. A yeast-related rash can involve skin folds and have small separate spots around its edge. Several conditions can look alike, so appearance alone does not identify the cause.

Change the nappy promptly after wetting or soiling, rinse with lukewarm water or use a gentle fragrance-free cleanser, and pat rather than rub. Allow short periods of air exposure when practical. Apply a thick protective barrier recommended for the child, especially after cleaning, and avoid talc, antiseptics, perfumed wipes, and scrubbing. Arrange prompt assessment if the rash is spreading, very painful, blistered, bleeding, oozing, crusted, or associated with fever. Seek urgent help if the child seems unusually sleepy, difficult to settle, or unwell, or if there are signs of dehydration. A clinician can check for infection, allergy, yeast, or another skin condition and choose treatment suited to the child’s age.

Ask what may be driving the rash, which cleanser and barrier are suitable, whether folds or nearby skin are involved, and how soon improvement should be expected. Mention recent antibiotics, diarrhoea, new wipes or nappies, and every product already used.
