What should you know about septic tonsillitis?

The word septic is sometimes used informally for tonsillitis that looks or feels markedly infected, such as when there is pus on the tonsils, a very painful throat, fever, or tender neck glands. It does not by itself confirm the cause or the right treatment.

Tonsils are tissue at the back of the throat that can become swollen and sore during an infection. Tonsillitis can be caused by viruses or bacteria; bacterial causes can include strep throat. A clinician considers the pattern of symptoms and examines the throat and neck to decide whether testing or treatment is appropriate.

Pain can spread toward an ear because throat and ear structures share nerve pathways. This can happen with tonsillitis even when the ear itself is not infected.

Arrange a GP consultation if the throat pain is substantial, you have fever, swollen glands, visible coating on the tonsils, or symptoms are not settling. Rest, regular fluids, and soft foods can make swallowing more manageable. Follow the dosing instructions for any pain medicine that is suitable for you, and ask a pharmacist or clinician if you have medical conditions, take regular medicines, or are treating a child.

Antibiotic treatment is used when a clinician believes a bacterial infection should be treated. Take it exactly as prescribed and do not use leftover antibiotics or share them with another person.

Seek urgent medical help if breathing is difficult, swallowing saliva is difficult, you are drooling, the voice becomes muffled, the mouth cannot open normally, or swelling is rapidly worsening. These features can signal significant swelling or a collection of infection near a tonsil and need prompt examination.

Get medical advice promptly if you cannot keep fluids down, show signs of dehydration, develop a widespread rash, or feel much more unwell. For a young child, reduced drinking, unusual sleepiness, or breathing effort also warrants urgent assessment.

Ask whether the findings suggest a viral illness, strep throat, or another cause; whether a throat test would help; and which pain-relief options are appropriate for your age and health history. If antibiotics are prescribed, ask what changes should prompt you to contact the clinic again.
