What should you know about scratching heat rash?

Short answer
Scratching heat rash can increase irritation, break small bumps, and raise the chance of infection, so cooling the skin and reducing friction are safer responses.

How scratching changes heat rash
Heat rash, also called prickly heat or miliaria, develops when sweat and heat irritate or block the tiny ducts that carry sweat to the skin surface. It may cause prickling, redness, or small bumps in areas covered by clothing or skin folds. Scratching adds friction to already irritated skin, which can make burning and itch more noticeable and create open spots.
Heat rash is different from every other [Skin Rash](/symptoms/skin-rash/). The pattern, trigger, and associated symptoms help a clinician decide whether another problem is present.
Cooling and skin protection
Move to a cooler setting, rinse with cool or lukewarm water, and pat dry. Wear loose, breathable clothing and change out of damp garments. Avoid heavy ointments, fragranced products, and vigorous rubbing while the skin is hot. A clinician or pharmacist can advise whether [Emollient Therapy](/treatments/medical-dermatology/emollient-therapy/) or [Topical Corticosteroids](/treatments/medical-dermatology/topical-corticosteroids/) is appropriate; do not apply steroid creams to large areas or broken skin without guidance.
When to seek care
Seek medical advice if the rash persists after cooling, spreads, becomes painful, or develops pus, warmth, marked swelling, or crusting. Fever, dizziness, confusion, weakness, or trouble breathing after heat exposure needs urgent assessment. Prompt care is also sensible for an infant with extensive rash or reduced feeding.
