Why the calf may cramp
Sciatica is irritation of a nerve that travels from the lower back into the leg. It may produce burning or electric pain, pins and needles, altered sensation or weakness; symptoms can reach the calf and foot. A cramp is a sudden involuntary tightening that often feels like a hard knot, and it can occur with fatigue, prolonged positioning, dehydration or other medical causes.

During a brief cramp, stop the activity, gently move the ankle and calf, and avoid forceful stretching if it increases pain. Note whether symptoms begin in the back or buttock, travel below the knee, recur with sitting or walking, or include numbness or weakness. Arrange assessment when cramps recur, disturb sleep, or occur with radiating pain so the cause can be examined.
When to seek care
Seek urgent care for a swollen, hot or red calf, sudden breathlessness or chest pain, because these symptoms may signal a circulation emergency. For possible nerve involvement, seek prompt help for new weakness, worsening numbness, inability to lift the foot, or loss of bladder or bowel control.
