What happens inside the hip

The growth plate, also called the physis, is a layer of developing tissue near the end of the upper thigh bone. During adolescence it can be vulnerable to forces passing through the hip. In SCFE, the ball of the hip joint remains seated in the socket while the neck and shaft of the femur move in relation to it. The result can be pain, stiffness, reduced hip motion, and an outward-turning leg. The change may develop gradually or follow a sudden worsening. A slip can be stable, meaning the person can still walk with support, or unstable, meaning walking is not possible even with crutches. The hip on the other side may also need assessment because growth-related changes can affect both sides.

What to do next

Stop weight-bearing and arrange urgent assessment by an orthopaedic team if SCFE is suspected. Do not encourage walking through hip or knee pain, sports, running, or jumping while waiting. An examination and hip imaging help show the position of the growth plate and guide treatment. Treatment commonly aims to prevent further movement, often with fixation using a screw; the exact approach depends on stability, severity, and the other hip. A clinician may also discuss safe transport and pain relief while avoiding medicines that are unsuitable for the child.

When to seek care

Seek same-day medical care for new hip, groin, thigh, or knee pain with a limp, limited hip movement, or a leg that turns outward. Go to emergency care if the child suddenly cannot stand or walk, has severe escalating pain, or develops a cold, pale, numb, or weak foot. These signs need prompt examination because continued weight-bearing can allow the slip to worsen.
