Scarlet fever progression: symptoms and changes over time

Early symptoms may include a sore throat, fever, headache, chills, or tender swollen glands in the neck. A fine rough rash can then appear, often beginning on the trunk or in skin folds and spreading across the body. The tongue may look coated before becoming redder and more prominent. As the illness improves, the rash fades and the skin may become dry or peel, especially on the hands and feet. Not every person has every feature, and other infections can follow a similar pattern.

Arrange medical advice when fever and a sore throat occur with a new rough rash. Offer frequent fluids, rest, and soft foods if swallowing hurts. Keep the person away from close shared contact while feverish or significantly unwell, and do not share cups, utensils, towels, or toothbrushes. Seek urgent help for difficulty breathing, inability to swallow liquids, drooling, confusion, fainting, severe weakness, or signs of dehydration. Request prompt review if fever is worsening or returns after improvement, the rash becomes purple, blistered, or very painful, or the person develops unusual swelling or reduced alertness. These changes are not expected simply because the rash is taking time to fade.

Ask which stage the illness appears to be in, whether a throat test is useful, how long to limit close contact, and which symptom changes should trigger urgent advice.
